Die clashes occur when the mechanical feed system fails to deliver a planchet into the striking chamber before the press completes its cycle, allowing the hardened steel dies to strike each other directly. The resulting transferred impressions — obverse design elements appearing in incuse on the reverse die and vice versa — create a permanent record of the event on every subsequent coin struck from that die pair.
