Identified as a probable Type 2 counter clash by Nathan McKinney, this 2008 Philadelphia Mint Arizona State Quarter displays raised vestiges of GOD and partial IN TRU from IN GOD WE TRUST on the obverse. The hypothesis holds that an intermediary piece of material — most likely metal — was struck over IN GOD WE on the obverse die, came loose, and migrated toward Washington's neck where it was struck again, counter clashing into the obverse die surface. Grease on the die may have facilitated keeping the intermediary material in place.
